Welcome to IBEW 309

The Trustees of Local 309 Health & Welfare Fund are pleased to notify you that beginning in 2003 you and your spouse were able to use your Physical Exam Benefit through the Health Dynamics Physical, every year instead of every other year.  You must complete all parts of the physical as deemed necessary by the attending physician. Any variance by the participant, could cause the claim to be waived for all members that are eligible to take the Health Dynamic Physical. The incentive for the Active Eligible Member and Active Eligible Spouse  could also be waived for not completing all of the physical or parts of the physical that the attending physican deemed necessary.


Effective January 1, 2008, Health Dynamics will administer the annual physical examination benefit.  The Fund will continue to pay 100% of the cost of the physical examination.  Eligible Active employees who get a physical will receive an incentive as follows:


First year:                                             $100

Second year:                                         $150

Third and subsequent years:                   $200


Effective January 1, 2014, the fund will continue to pay 100% of the cost of the physical examination for Eligible Active spouses, and they will receive an incentive as follows:


First year:                                             $100

Second year:                                         $150

Third and subsequent years:                   $200


If an active eligible employee or spouse does not take the Health Dynamics Physical in consecutive years at one of the locations that are listed at the bottom of this page, the incentive payout will revert back to the active  eliigible employee and spouse incentive of $100 for the first year again. If the Health Dynamic Physicals  are consecutive after the first year,  and you are going to the health dynamics physical for the second consecutive year, you will get the $150 incentive payout. If the active eligible member and/or spouse due the health dynamics physical for the third and subsequent years after three or more years the incentive payout will be $200 until the memeber misses one year and it would start over again. The active eligible employer and spouse they will have to buid the i second year of $150 ,and the third and consecutive year or more years the incentive paid is $200, and they will stay at $200 until they miss a consecutive year. 


The incentive will be paid by the Fund Office with a paper check. The incentive check will be issued in the members name for the employee and for the spouse. If the employee and spouse each attended the physical, each member and spouse will be issued two seperate incentive checks, both cheks will be in the employees name. Once Health Dynamics mails the claim in to the Fund Office to process, the checks will be issued the quarter after the claim is paid to Health Dynamics. (ex. if the employee, spouse or both have the Health Dynamics Physical in the end of Jan, then we receive and process the claim in March, the incentive check will go out in middle of April. In a few occassions we will not receive the claim from Health Dynamics for a few months, but as soon as we receive the Health Dynamics claims, we process them in the next set of claims that do we send out. Receiving the Health Dynamics claim at a later date could affect when the incentive checks can be cut and go out.)


Retirees are not eligible for the incentive.  However, retirees and Medicare eligible retirees are eligible to receive an annual physical paid at 100% by the Fund Office.


This benefit is only available through Health Dynamics and the exam is performed at the four facilities listed BELOW.


The PSA test is being offered to all males. 


Call one of the area hospitals listed below and ask to start on the Health Dynamics Program through IBEW 309.  You will need to set up an assessment appointment (preferably in the morning).  Your assessment appointment will take approximately 2 hours so plan accordingly.

Complete blood chemistry analysis, including glucose and cholesterol (41 elements)

Urinalysis (16 elements)

Body fat composition measurements

Resting blood pressure

Height and weight measurements

Pulmonary function test (lungs)

Strength evaluation

Flexibility testing

Twelve-lead EKG recording

Cardiovascular fitness test – bike or treadmill

Physician directed examination

Pap smear for women (upon request)

Colorectal cancer screening (multi-slide available)

Breast screening mammography, according to ACS guidelines (upon request)

PSA test for prostate cancer, according to ACS guidelines

Chest X-ray (OSHA protocol only)

One-hour wellness coaching/consultation
